The negatives of poor community of care are housing instability as individuals fierce and increase risk of homelessness often ending up in shelters or unstable environments. This makes maintaining employment and appointments impossible
Health and addiction risks a lack of continuity means missed medications and appointments. This often causes symptoms to worsen and without ongoing care. The risk of relapse and overdose is high.
Psychological and social decline a breakdown and trust with services leading to giving up on support and feelings of hopelessness. This place is immense strain on families and increases in stability within communities.
Socio-economic barriers so without assistance for CVs job training or benefit set up individuals fall into poverty and may rely on unsafe income sources
The combination of poor mental health, lack of housing, and returning to old habits significantly increases the likelihood of reoffending.

Homeless shelters often have very limited availability, and many prison leavers are turned away due to offence histories or probation restrictions. Short-term "night-by-night" accommodation makes it difficult to maintain a consistent address for employment or healthcare.
